What size does an Ohio land surveyor seal have to be?

OAC 4733-23-01 governs the Ohio land surveyor’s seal but does not prescribe a diameter. Sizes quoted for Ohio elsewhere generally come from stamp suppliers’ catalogues rather than from the rule. What the rule does require is that your name and license number stay clearly legible.

Does an Ohio surveyor's seal read "Land Surveyor"?

Surveyors seal as "Registered Professional Surveyor". Ohio does not put "Land Surveyor" on the seal. (OAC 4733-23-01)

Does an Ohio surveyor's seal carry the registration number?

The seal rule does not call for one. It requires a seal of the design authorized by the Board bearing the registrant's name together with the legend "Registered Professional Surveyor", and names nothing else — no number, no state, no date. The registration number is mandatory somewhere else: the minimum standards for boundary surveys require the plat to show the surveyor's printed name and Ohio registration number, along with the signature and seal. A surveyor who treats the seal as carrying the number will leave the plat short of an element the standards require. (OAC 4733-23-01(A); OAC 4733-37-05)

Must an Ohio surveyor's seal reproduce on copies of a plat?

Yes, and that is a real constraint on how the seal is applied. The minimum standards require the plat of survey to show the surveyor's printed name and Ohio registration number, signature and seal "in a form which may clearly reproduce on any copies which may be made of the original drawing". An embossed impression with no ink behind it satisfies the seal rule but disappears from a photocopy or a scan, which is exactly what the plat standard forbids. (OAC 4733-37-05)

Which dates go on an Ohio surveyor's plat of survey?

Two different ones. The minimum standards list "the date of the survey" as an element of the plat itself, and the seal rule separately requires the work product to be stamped with the seal and signed and dated, or to bear a computer-generated seal with an electronic signature and date. The date written beside the signature records when the surveyor sealed the drawing and is not a substitute for the date of the survey, which is the fieldwork date the plat has to state in its own right.
